Requests may include Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), Direct Support Professionals (DSPs), allied-health professionals, and other applicable healthcare or support professionals. If you are unsure which role is appropriate, tell us what support you need.
Submit a short service request
Use the online request form to provide your organization and business contact details, the professionals needed, general service location, timing, and a brief description. Do not include patient or resident information.
Review and follow-up
SAMFAS reviews your request and contacts you regarding availability, qualifications, facility requirements, and next steps. Any further information needed for contract preparation is requested separately after review.
