The steps below describe the information applicants will be asked to provide. No application can be submitted through this branch.
Professional qualifications and readiness
Provide the credentials, licenses or certifications, role experience, and other qualifications relevant to the services you intend to provide. Professionals should also be prepared to work reliably and respectfully in care environments.
Availability, service area, and care settings
Include your availability, preferred service area, and the healthcare or support-service environments in which you are prepared to accept responsibility.
Standards and compliance documentation
Submit required business, insurance, workers’ compensation, credential, or other applicable documentation through SAMFAS’s online onboarding process. SAMFAS may request additional information during review. Professionals are expected to follow applicable care plans, client or facility procedures, and relevant health and safety standards.
Review and referral readiness
SAMFAS reviews onboarding information, qualifications, and any additional requirements before considering suitable opportunities. Submission does not guarantee placement, referral, assignment, or engagement.
